I've got Howard's recipe for baking soda fungicide, but how do I mix it for use in a sprayer that attaches to my hose, so I can spray my big persimmon tree?

Just mix the baking soda with water in your hose end sprayer. You want to the product to be in solution so that the sprayer can draw it out. Do not use too much baking soda because you will leave sodium behind. You should really use a product called Remedy, potassium bicarbonate, it will leave potassium behind rather than sodium.
